The next H1N1 Influenza vaccination clinic is Tuesday, December 1st from 1 - 7 pm (or until vaccine is gone) at Fair Park Pavilion on Hwy P in Jackson. Those under 18 years old must be accompanied by an adult and have a vaccine administration form signed by a parent or legal guardian.  The administration form is on the Health Department's website at www.co.washington.wi.us/chn and can be filled out on the computer, printed, signed by the parent and brought along to clinic.  The supply of vaccine is still not reliable enough to plan school clinics.  Additional clinics at the Health Department are listed on the attachment below.

The following groups will be vaccinated at the December 1st clinic:
Individuals aged 6 months to 24 years old
Pregnant women
Close contacts of infants less than 6 months old

Donor's Choose Website
Donor's Choose is a wonderful website teachers can use to post worthwhile projects for their classroom, school or district.  Over the summer Mrs. Bauter posted a project in hopes of ordering additional supplies to help with our current reading program.  A number of donors have contributed, but a few more are needed in order to fully fund this project.  If you are interested in checking out the project please visit: Mrs. Bauter's Donor's Choose
